Clinical Social Worker Essential Job Duties:

The Clinical Social Worker is responsible for performing assessments appropriate to functional status, diagnosis, care setting, age, development level, and cultural and religious orientation in a timely manner. The candidate will also identify and prioritize the need for further assessment based on the patient's clinical presentation/diagnosis, care setting, desire for care and responses to any previous care. The Social Worker collaborates with Case Managers on discharge planning.

Qualifications

* Two (2) years social service/discharge planning/case management experience, preferably in an acute care hospital
* Completion of an accredited Social Work Program
* MSW Required
