Job Description:
A company is looking for a Care Coach to provide support and
guidance for individuals in need of mental health care.
Key Responsibilities
Identify individuals needing care coaching and provide motivational
interviewing and health education
Facilitate access to care and community services, and refer members
to appropriate resources
Utilize care management software for documentation and maintain
confidentiality of personal health information
Required Qualifications
Background in Social Work, Psychology, Public Health, or a related
field
1-2 years of experience in a mental healthcare role preferred
Knowledge of medical terminology and understanding of mental health
complexities
Proficiency in CRM software and Google Suite
Experience working with diverse populations seeking mental
healthcare is preferred
