Job Description:
Description RN Care Manager for our Care Management team at
Providence St. Joseph Hospital of Orange in Orange, CA. This is a
Per-Diem position that will work 8-hour Day shifts. Care Management
is a collaborative practice model including the patients, nurses,
social workers, physicians, other practitioners, caregivers and the
community. The care management process encompasses excellent
communication, both verbal and written, and facilitates care along
a continuum through effective resource coordination. Providence

them. Required Qualifications:
- Coursework/Training: Graduate of an accredited school of
nursing.
- upon hire: California Registered Nurse License. Preferred
Qualifications:
- Bachelor's Degree in Nursing. Or
- Master's Degree Related field.
- upon hire: Certification in Case Management (CCM, ACM).
- upon hire: Specialty Certification in Oncology, Rehab,
Pediatrics, Transplant, or Wound Care.
- 3 years Experience in utilization management or case
